1 A sole proprietor/sole proprietorship is an individual, and as such, is eligible for a single NPI number. The sole proprietor must apply for the NPI number using his or her own Social Security Number (SSN), not an Employer Identification Number (EIN) even if he/she has an EIN.

Prescribed doxycycline, a stated allergic to antibiotic causing a re hospitalization with drug induced elevated liver enzymes, dehydration, severe nausea, weight loss & pneumonia. Greatly hampered my knee replacement recovery. Also, ironically stated to follow up with a PCP I hadn't seen in 15 yrs. Sadly, she did not read my chart. Zero stars should be an option
