Golden Point Solutions, LLC needs to know that I was told by the Revive Medical Center, LLC,in Haleyville, Ala. Dr. Collins asked me if I wanted bloodwork done, and told to me the cost of the test would approximately be. He did not inform me that this cost was in addition to and independently billed from what Revive Medical Center, LLC also charges for the drawing of the blood. It is because it was not made clear to me that Revive Medical Center charges to draw the blood, much less inform me that the Golden Point Solutions test was not apart of the total cost of the medical I visit, which I paid in full, that I was denied the opportunity to make an informed decision about my health care.
